A Case of Primary Nasal Tuberculosis


Abstract
Primary tuberculosis of the nose has been reported to be rare in Korea because of well planned tuberculosis control and anti-tuberculous therapy.
A 20 year-old girl, otherwise, relatively healthy, was consulted to the department of E.N.T. fromM the department of Dermatology because of tulcerated lesion on the cartilagenous portion of the nasal dorsum with the chief complaints of nasal obstruction, foul odored bloody discharge with fregent epistaxis and crust. formation for one year.
Nasal examination revealed a ulcerated lesion measured about 1. 0 X 1. 0 cm in size on the cartilagenous portion of the nasal dorsum and a purplish red coloured broad based bulging mass measured about 1.5X 1.5 cm on the anterior superior portion of the nasal septum covered with dirty crust on both sides.
A.F.B. stain and fungus culture from the intranasal lesion revealed as negative but histopathologic examination of biopsied specimen from the nasal dorsum showed as chronic granulomatous inflammation consistent with tuberculous infection.
Radiological examination of the P.N.S. shows mucoperiosteal thickening in the both maxillary antrums suggesting maxillary sinusitis but the chest X-ray showed negative findings. She was diagnoed as primary nasal tuberculosis and treated with S.M 1. 0, I.M. every otherday, I.N.H. 300 mg and PAS 9 gram daily with a good result. After 60 days treatment, nasal lesion is cured with a faint skin discoloration on the nasal dorsum.
